Forms for Veterans Benefits

Students interested in applying for VA benefits at Collin College can use the following forms to apply for education benefits and visit the Veterans Resource Centers on campus for more information.

Forms: Veterans

• Collin College Veterans Affairs Application (VAAP)
  
  
• Form 22-1990: Application for VA Benefits - Proceed to www.va.gov and follow the instructions to fill out the application for educational benefits. Print a copy of the confirmation page and submit to Collin (vrc@collin.edu) with other paperwork requirements.
  
  
• Form 22-1995: Request for Change of Program or Place of Training - If changing schools, proceed to www.va.gov and fill out the form. Simply indicate that you are changing schools (use Collin College, 2800 E. Spring Creek Parkway, Plano TX 75074 address). Print a copy of the confirmation page and submit to Collin with other paperwork requirements.
  
  If changing degree plans while at Collin, submit a degree plan request form or certificate program request form to the Veterans Services office.
  
  
• Degree Plan Request Form - This is needed when you are filling out initial paperwork and when changing your degree plan.  Please only select one degree program.
  
  
• Certificate Program Request Form - This is needed when you are filling out initial paperwork and when changing certificate programs. As a reminder, you can only select one certificate.

 

Forms: Dependents

• Form 22-5490: Application for Survivors' and Dependents' Educational Assistance - Proceed to www.va.gov and follow the instructions to fill out the VONAPP.  Print a copy and submit to Collin with other paperwork requirements.
  
  
• Form 22-5495: Request for Change of Program or Place of Training Survivor's and Dependents' Educational Assistance - If changing schools, proceed to www.va.gov and fill out the form. Simply indicate that you are changing schools (use Collin College, 2800 E. Spring Creek Parkway, Plano TX 75074 address).  Print a copy of the confirmation page and submit to Collin with other paperwork requirements.
  
  If changing degree plans while at Collin, submit a degree plan request form or certificate program request form to the Veterans Services office.
  
  Dependents can use the above VAAP and Degree Plan Request Form or Certificate Program Request Form

• To view your degree audit audit log into CougarWeb and Select the Cougar Compass Audit feature under the Student tab to run your current degree plan.
